That's Profit UI Reborn slightly modified with wide screen, as far as I can see.

Indeed, which is probably around 2 weeks from now.

Edit: The new poisons only had their proc rate increased to 5, with the Elixir providing 4 DA instead of 2. (The ones mentioned in the Test update notes)
