Default Hit Rate issues

As of late on my bruiser my parses have ate shit and died and im confused to say the least. I've had plenty of battles where shit has gone fine and dandy and I parse 5-8k consistently depending on what I'm doing. The complete opposite happens on some battles where suddenly i see my parse dragging around 2.5-3k. Having gone over ACT several times over now on several occasions even against trash I'm some how hitting 60-65% hit rates, and this is when I'm using offensive stance.

I'm usually not tanking the mob unless MT goes down and OT is preoccupied. I'm not in the best gear ever but I do have a few nice pieces this expansion. Myth + nightmare wraps. My question is has anyone else had this problem and anyone have any idea how I could fix it? I have the 5% accuracy AA. Generally speaking I get weapon boon as well so its not like I'm low on crushing skills in raid. Master stances as well.

I've actually had Higher hit rates in defensive stance some times compared to my offensive stance. Its really kinda borxing me.

I personally do not have this issue but I have seen something like this come up.

We have 2 brigs in raid even though one would have better melee skills and both fully mastered The first one will come out with a low 70% hit rate and the other in the high 80's. No clue whats going on.

Only time I have a crappy hit rate is in heroic instances. In raid I am usually in 90%+ with dirge.

Things that could affect your hit rate might be if you have different dirges, perhaps one has the accuracy bonus from set gear and one does not, or maybe you have a ranger giving accuracy bonus as a temp. Additionally if you have a monk in raid their accuracy stacks with ours, and we all know about a warden's single target.

Same here, i always complain about my hit rates in heroic instances.
As for raids our raid leader is smart enough to put me in groups where even in defensive i can have a high hit rate against orange mobs.

But yeah, in instances it's crap, i really need a dirge and a warden or templar with it to actually have a descent hit rate. Since i got the +5 raid accuracy it seem to happen less though.
Misses piss me off.

I'm necro'ing this thread - mostly to get some visibility on the long-standing nature of the problem.

Hit Kurn's tower last night - was tanking it. Running offensive stance for regular mobs, mid on most named, defensive on final 3 named... fabled epic, +10 crush in charms total, +6 crush on ring, +6 crush on cloak, dirge in group...

Zonewide hit rate for slash/crush (using greenmist for miti proc) -> ~37% Zonewide dps; right at 2k. Now, with coercer, dirge, swash in group and some clever juggling of abilities, aggro was locked for the entire run - but it was a royal PITA to keep it and required THREE aggro control classes (got lucky that group composition just happened to have that).

So - what's the verdict here? Should I just assume I'm never going to be able to hit anything unless I'm in offensive w/all dps gear leaning toward +crushing?

I have the damned 2 MC +5 weapon stat earrings from RoK questing still that I break out, earring of icicles, wasted wilderness set - you name it, everything I can find with +crushing. I've gone into zones w/over 500 crushing and walked out with < 90% hit rate. What the hell?

repeated edits to try and get carraige returns to actually... work

Herioc zone mobs appear to have gotten ALOT of uncontested avoidance since RoK for some unknown reason, Apparently a friend that does the cowardly greyed out instances says the hit rates are terrible even for an 80 to hit a level 50 TSO mob.
