Like every forum has one so even though it's a couple weeks late we're going to have one, too.
I use the mouse pretty much exclusively when casting stuff and it works well for me.
So what's your setup and do you use the mouse or keyboard primarily when casting your stuff?

Pretty simple.. attacks followed by debuffs on the first line. Buffs and stuff I don't use as frequently on the bottom bar. The debuffs start on bar2 so most times I shift-2 and then 1,3,... it or I use the mouse to click debuffs while a 1,2,3.. the attack keys.
As far as the chat windows.. left is guild/group/say/shout etc, middle is damage out, and right is damage taken and the public/private channels.
I generally mix between the keyboard and mouse.

I use the keyboard and the mouse. Hotbar shortcuts are active on the top 3 rows. Frontal attacks on bar one (Assist macro in slot one), positional and stealth attacks on bar two, rezzes and debuffs on bar 3. By keeping my left hand on WSAD while moving with the mouse, I can get to anything very quickly.
Group and raid windows are not visable in the shot, but group window is stacked vertically to the right of the hotbars, raid window is in a square above my spell effects window. If someone dies, I mouse over to them, click to select and smack Control + 1 or Control + 2 for the rez.
Other stuff that works well for me:
Lucky Break + Tarvens + Verliens + Ludas for a rather nasty HO + DD Nuke combo. Lucky Break and Tarvens to start and push the HO through the first step, then use Verliens to flip it for another shot at Bravo's Dance, or to get Swindlers Luck if it's not already up. I've had this combo crit for 3k+, plus you get the HO off as well. Even if all you get is Ringing Blow, it's more dps.
Alt 1 and Alt 2 or 3 for a fast stealth and Garsins or SoD
If solo, Cheap Shot and then jump over the mob while hitting Shrieking Stab or the Mastery Strike.... can be effective if you've pulled aggro in a heroic instance too... not that us dirges would know anything about that

Mostly use mouse for casting, then 2 ---> 3 ---> 4 for quick HO.

I've spent a lot of time playing around with my UI, and this works pretty damn well for me. All my HO triggers and advancers are in the same row for quick and easy HOing, my assist key is #1, followed by Daros and Disheartening for quick and easy debuffing.
9 is Cacophony with a built in macro.
I like to have everything near to do as little clicking as possible. I keep SoD and Garsin's right next to my invis button.
Another key thing to note for me, is that I'm right handed so I only keep my left hand on my keyboard at any one time. Thus 1,2,3 are easy to reach, but after 6 it goes to shit I need to click. Waning Moon Cutter and Longsword of Perfection and so on down the line.
I keep all the shit I'll never need except for that one random rare time on the right hand side way away from everything else. No, Evac is not there, and no it won't be, because I'm primarily raiding with this toon and if I'm the last person alive to evac in a raid instance, pretty sure I'd rather just die anyway. Oh, and if you're thinking it's cause I fucked up before, you're mistaken
I have an auto attack hotkey next to my ranged hotkey incase all my CAs are down to re-engage me in melee attack after a knockback or helping with adds on some such encounter like Mayong or somethin'. That's pretty much it, feel free to ask questions

haha yeah... not like it matters. he has what? 2 good loots? I really wish he wasn't nerfed so hard, he used to be kinda fun, now he's lika ^^^ with about 80x more hps
