In your brother's case its due to a bug that he has not patched. In the farmers case there is a handy dandy program that some banned idiot posted on flames which is able to consistently repeat this issue. For ppl that are unpatched and having that bug its fairly inconsistent in reproducibility. For these assholes its totally consistent and is using a program to ensure they can cheat the game to make plat.

How do you not patch a bug? I don't ever remember being given an option to not download the latest patch every time I log in. What am I missing?

They have to let the client decide some things or there would be so many lag issues the game would be unplayable even on the fastest of internet connections. Timers in particular, but also any logic that deals with positioning. The server just needs to do sanity checks on those values to be sure no one has hacked them.

No, no contradiction. the sanity checks don't have to be given any control. They can be setup to flag problems so real people can investigate and take action after the fact. Or, if they do take action, then they'll just shut down a connection thats getting too much lag (give them the benefit of the doubt here). But they do not have to slow anything else down until a problem is detected.
