It's Infusion for sure. Basically it says it will strike "a target in area of effect", essentially saying that it will strike that target once per beneficial spell. However, the target it strikes is based on which mob (assuming there are more that are either out of combat, or in combat with you) is closest to where you stand. Therefore, if you stand near a non-aggro mob, but cast a beneficial spell while in combat, it will proc on that mob. That's why, if you're using it to dps, you need to stand pretty much on top of the mob you're fighting, unless there aren't any other mobs around, or it's the only mob in the encounter (I stand on the mob mainly so I make sure it's hitting the debuffed mob in raid :P).
