As Snark said, it has to do with an intelligent player. Although it's too hard to ever tell who is the 'best' at there class, there are usually several that stand out.
A player who can take advantage of certain situations and bend them to improve themselves is certainly going to stand out from others. I'm not speaking exploits, but say from a DPS standpoint being smart enough to take advantage of encounter types/AE types and using your abilities in a manner which allows you to dodge AE and maximize melee/ranged auto attacks will put you out in front of the rest. As any good Melee based DPS will tell you, this is true. Another point would be a player knowing exactly how each class will affect their DPS as a whole, and how buffs and buff timers will affect their routine. Any DPS class that couldn't tell you exactly what which classes do for them, and what would be better than which class, isn't capable of reaching their classes maximum ability. I only speak of DPS because that's what I know best, but each class has their own niches that they can take advantage of to improve their output and the people at the top of their classes know exactly what those are.
