Well not only create an MMO for peeps who are hardcore, but create a damn good one. It really is possible actually to make a game with a good hardcore "feel" and still have casuals able to play the game. You just gotta bring back some unpopular things. I for one would bring xp loss back into the game when you died, including the "un-ding"
Doing that has the benefit of making everyone continually experience your heroic content in some form. Make dungeon crawling a good form of xp (honestly, the horrible xp that was present in KC was a big mistake, imo) and make more contested dungeons with loot that is decent for heroic content, with a chance , even if it's a .001% chance, to drop something really nice that would even be desirable by most raiders, or at least comparable to raid drops.
Anyway, sorry to rant...but a hardcore MMO I think would do very well. But then again, the sad state of affairs that any game with actual risk/vs reward, actual challenge of play even at lower lvl's, and actual progression would be nerfed within 3 months because people would complain it was too hard
