I raid in top setting, maxxed out all the way. 60 - 100 fps.
Core 2 Quad Q9650 Extreme, 8GB of DDR3, Asus Striker II Extreme, Velociraptor 300gb HD, GTX 280 with Tieton Waterblock OC'd like a mofo.
I drop SOMETIMES in bad situations, but I think that's purely due to memory leaks and other bullshit in the game that just needs fixing.
You can do Balanced with a LOT cheaper of a machine though, to be honest.

hhmmm.. i have the same cpu as you with liquid cooling cpu, Nvida 780i MoBo, GTX 260, 8 gigs Musking Extreme ram.. and a WD 74g 10k rpm HD.. and i cant raid in balanced i get about 8fps. what am i doin worng? have to raid in Profit ui raid setting

Yeah I Call bullshit, show me 90 fps with 24 toons blasting an epic with 4+ spell results each with all sliders pushed. Screenshots <3
Me? I have never used the custom profile settings because I like to set the options myself...so I have no idea wtf you mean by balanced. I turn alot of flashy stuff off so it cast hopefully hang around 30 - 50fps
(core 2 duo 3.0, 8800GT 512, 4gig ram)

And what resolution?
I'm able to run Balanced but can't exactly raid with all the spell effects unless you want your eyes to bleed. 30 FPS @ 1920 x 1200 with a 6600 @ 3.6 / 2GB / 8800 GT

I call bullshit too.. i got quad core with 4gb ram and nvidia 9800 gtx and i'm still getting like 10 fps at balanced..
Although, if i take shadows off can have at least 10 more fps but nah.
