I have myself my 'main' 70 Ranger with 100 AA's all pimped. Then I have a bunch of 70 with 100 AA's that I use to help the guild out.
We are not hardcore and well its far easier to find a dps class than it is a support class or a healer, hence why I have a Mystic/Coercer and Dirge all 70 with 100 AA's.
It seems that these days nobody wants to play a healer and if they do, they get bored after a few months and decide they want to switch, what can you do

I have a lvl 70 coercer (75ish aa), level 70 pally (50 aa), and a now-65 necro who is my main. I just play what is fun to me, and my alts are there for the times when you just need a coercer or a tank.

I raid on my ( betrayed) templar mainly( shes OOBAH!) but I also have a 70 inquisitor( <3 Battle cleric!) and a 70 necro ( old main now spoiled rotten alt) and a 60 coercer im leveling up with also ( back up buff bot if theres room wherever I go lol) I have a 54 high elf defiler who rocks for farming lower levels for sure! I also have a lvl 45 sk(50 alchy) I am working on also I lub her too. Also I have a 48 Dark Elf monk she is my necros twin.. and and ............I have 3 accounts lol I could really list them all but you get the picture

I'm not good at splitting up time. I take a character and focus on it until I get to 70 bored or sick as hell of handing out hearts in raids etc. I dual box on ocassion but it's somewhat a pain. I'll generally consider my main the one I'm devoting most of my time on. It changes every once in a while. Keeps interest in the game I suppose.

I consider my two primary played toons as mains. I two box a 70 Sk and 70 Defiler, while the SK is my "main" for guild/raiding purposes, the Defiler was my main in my previous guild. Both are almost fully T7 raid geared, so can play either, depending on which toon Santus Immortalis needs.
All the rest of my various toons...46 Conj, 33 Brig, 20~ Bruiser...are what I consider alts.

I consider my Warlock my main. My Dirge (Post EoF char), has almost as much AA and will soon be the same level. (Likely within the next couple days)
Calling my second character an Alt, really isn't a big deal to me. Really, when you look at it .. it is an "Alternate" character.
The main reason I refer to it as an Alt, is I haven't farmed nearly as much playtime on it .. and it has a quarter of the quests completed on it.
Should it's completed quests/gear/etc surpass my main? I would likely start calling the Dirge my main. My Warlock has enough done however I can't see that happening anytime soon.
For me at least, the designation of "Main" is purely to distinguish which character has more overall work put into it.

If "Main" means more work put into them, then Rhoslyn is my true main. Nagafen may have more people than Befallen, but I like both servers.
Otherwise, I like reading people's opinions on it, so keep posting!
