I usually start with a ranged auto attack, and then run through a good chunk of the melee CA's (especially debuffs, bloody reminder, ranger's blade) while allowing ranged AA to fire between - then time it right so ranged AA goes off right after casting focus aim, and then run through my ranged CA's ending with rear shot (joust out while it casts) - coverage - sniper/hidden/blue aoe(can't remember the name off hand, but you can joust back in while this one casts).
If you do it right, focus aim should end just shortly after casting your last ranged CA (or you can cancel it out early) Then go for honed reflexes/instincts and move through some more melee, bloody reminder should be back up, etc.
ALWAYS get your ranged AA's in between CAs, never melee AA, and use ranger's blade and bloody reminder when they're up (well don't use bloody if fight is going to end in the next 10 seconds).
Of course this is based on a long fight, and is more or less the ideal. Adjust accordingly
