Sorry to be an utter n00b but what am I missing here. I thought this only proc'd off a melee attack. If so should I be up there with the melee on named?

Umm, why would you not be meleeing? It's extra DPS. Regardless of your weapon, you should be meleeing. With Yeb-Soth's, even more reason to do so.

Venril = Only when Sanctuary is up.

a)so long as meleeing doesn't distract you from healing.
b)so long as you can survive the AEs/Damage shields and make sure you're not getting frontaled, riposted.

If you can accomplish both A and B without affecting your performance at maintaining the MT then club away. If you can't, for the fights where you're going to hang back, find a different 1hder.
