A couple bugged spells...
Purity/Tranquility is not healing for as much as it says in spell description. Initial heal is about 80% and each other heal is about 55% of what is said in description.
Rebirth AA : Falsely triggers sometimes when an emergency anti-death triggers.

I'm betting you are noticing Purity to heal less on the second heals because of your +Heals being frontloaded.
Death Interventions have been bugged since the release of this game.
There is no proper coding to tell the game which should trigger first or why. That being said of course multiple-trigger death interventions will have minor issues. Most of the time, if its just your death intervention that is up, and the person you casted it on noticed it only triggered once, the wonderful world of lag came in to fuck with your spell.

Purity's description is telling me it heals for 1019 and 1019 per thing it dispells. However it ALWAYS heals for mid 800s then mid 500s for every thing that it dispells.

I noticed you are on Nagafen fine sir.
Could this discrepancy you are seeing between the displayed value of the spell and actual value of the spell be due to the fact you are in PVP combat?
Its probably a combination of what Phay said (+heals front loading) and you being engaged in some sort of PVP combat that makes your heals gimp.

First off, i'm new to the warden and healing community. You'll have to excuse me for being retarded in this area.
Just to add to what the OP said (and im PVE)... When i add, for example, a +45 heal amount item, it shows up adding around 9 to Purity. Is this b/c it's spreading the heal amount from the initial heal thru-out the possible heals (if they had nox, arcane, elem, and trauma on them)?
Edit -(Initial heal - 9)+ (nox - 9) + (arcane - 9) + (elem - 9) + (trauma - 9) = +45 heal amount?

Huh... + to healing doesn't affect purity as far as hovering my mouse over the spell shows. Now if it actually affects it through act I couldn't tell ya, never checked it out.

Well, I don't have much + to healing gear, and my Purity was at 910...I added a +45 heal adorn for neck, and magically Purity was up to 919....and yes, I was just hovering my mouse over the spell on my hotbar. /shrug
Actually, that's exactly what it's doing now that I think about it, and stop doubting myself...LOL.
+ to heals on Purity is divided by 5 b/c of the initial heal, then the 4 possible cure heals....

Okay That's what the issue was, I had tooo much + to healing on and had it way over capped. Had to remove +375 to healing just to see it move down, mines capped at 894 though, do you have the master or something? Either that or soe just hates me and gave me a jacked adept 3 of purity :P
